Thank you for taking the time to register your household's information with our parish's office. Please print the form below and return it to the parish office. Copies of the registration form are also available in the white Welcome Envelopes in the narthex.
Parish registration forms can be dropped by the front office during regular office hours or emailed to our Parish Administrator.
